Many BienPoMD facial fat-graft plans combine two preparations from the patient’s own harvested fat. Microfat preserves small fat parcels that can replace lost facial volume and soften selected contour transitions. Nanofat is more finely processed and is not relied upon for meaningful volume; it is used for its stromal and signaling components to support skin and tissue quality. Using both can address structure and surface quality in one individualized plan, while avoiding a synthetic volumizing implant. Results still depend on anatomy, technique, healing, and variable fat survival, and additional treatment may sometimes be needed.

What Is Facial Fat Graft Transfer?

Facial fat graft transfer moves a patient’s own fat from a suitable donor area to selected regions of the face. After harvesting and preparation, small amounts are placed in carefully planned layers to restore volume, soften contour transitions, or support facial balance. Common treatment areas include the temples, forehead, cheeks, under-eyes, nasolabial folds, lips, and selected acne-scar areas.

Who May Be A Candidate?

Suitable patients have facial volume loss or a specific contour concern, enough donor fat, realistic expectations, and a medical assessment confirming that fat transfer is appropriate. The plan must account for skin quality, facial proportions, previous procedures, and whether awake treatment or IV sedation is safer and more comfortable.

Microfat and Nanofat

Microfat is prepared to retain small fat parcels for structural volume replacement. Nanofat is more finely processed and is used primarily for tissue-quality support rather than projection or bulk. A combined approach may address both facial volume and the quality of the overlying tissue, but it does not guarantee a particular degree of rejuvenation or permanent fat survival.

Procedure and Recovery

Swelling and bruising can affect both the donor site and treated facial areas. Early fullness is not the final result because swelling settles and some transferred fat is naturally reabsorbed. Final retained volume varies, and a staged or additional treatment may occasionally be recommended.

Risks and Limitations

Possible concerns include asymmetry, contour irregularity, infection, oil cysts, calcification, undercorrection, overcorrection, fat necrosis, donor-site changes, and rare but serious vascular complications. Facial fat injection requires detailed anatomical knowledge and careful technique.
