Concern

The patient was satisfied with the bridge and wanted focused refinement of nostril width, flare, and shape.

Assessment

The nasal base showed alar flaring, alar overhang, and a broad nostril base. Preserving the bridge and identity was central to the plan.

Technique

Focused correction of alar flare, alar overhang, and nostril-base width while leaving the bridge unchanged.

Postoperative timing

Intraoperative and immediate postoperative views.

Realistic interpretation

The base appears more refined, but sutures, swelling, redness, and temporary asymmetry are expected. Final shape becomes clearer as healing progresses.
